School Districts and Administrators
- Formal Complaint Form (District or Private/Charter School Use Only) - Use this form to refer a complaint to OSPI for investigation.
- Note: Under RCW 28A.410.090, OPP cannot accept the Formal Complaint form unless it is signed by a superintendent or chief administrator.
