Chapter 8: SCAP Closeout (Form D-11/D-12)

Form D-11/D-12 (Application for Release of Retainage)

For projects funded through the School Construction Assistance Program (SCAP)—see Chapter 3: School Construction Assistance Program (SCAP) Eligibility—upon final completion of the project by the contractor(s), the architect/engineer shall inspect the project to determine compliance with the construction documents. When compliance has been verified, the architect/engineer shall issue a certificate of completion to the district along with a statement of the completed project square footage. The district shall then formally accept or reject the project. Release of retainage, whether held in cash or secured by a bond in lieu of retainage, may occur only after OSPI has received the required closeout documentation and issued Form D-12 authorizing release.
