Requirements

  • Program approval (contact your institutions certification officer for approval process)
  • Submission of fingerprints for a background check is required, if a valid certificate is not already on record

NOTE: If you are an out-of-state applicant, you will need to have you institution complete Form 4050E. In-state applicants, please contact your institutions certification officer for more details.
